Clearances

Health Clearance–As mandated by the Code of Virginia (22.1-300) you are required to submit a signed and dated statement from a licensed physician or health department certifying that you are free from communicable tuberculosis. Please submit this document addressed to the CoE Director of Clinical Experiences and Partnerships in a sealed envelope via Ms. Tember Smith (Fredericksburg Campus) or Ms. Terry Luttrell (Stafford Campus). Candidates completing their Internship during Fall-Spring 2012- 2013 will be able to complete this process during their orientation day on 16 August 2012. Health Clearance verification will need to be updated every 12 months while enrolled in a UMW CoE licensure program.

Criminal Background Clearance—verification through the Virginia State Police that the individual has a clear record in relation to the Child Protection Act. Only the student, the CoE Director of Clinical Experiences and Partnerships, and the CoE dean will have access to the VSP reports. The initial finger print clearance requires a payment of $26.00; because this is an academic program requirement students’ financial aid/scholarship funds can be used for this fee. Forms and schedules for processing the clearances will be posted on the CoE website shortly and notification will be sent to students when these are available. The background clearance will need to be updated every 12 months during active enrollment in a CoE licensure program. VDOE: Child Abuse Recognition & Intervention Training—

Follow the instructions, complete the training and test; print the certificate and submit it to either Ms. Tember Smith (Fredericksburg Campus) or Ms. Terry Luttrell (Stafford Campus). The module is available at no cost and the one time submission of the certificate will satisfy the requirement for all practica and internships completed during your program of study.
